Our Approach

To access Level supervision from a trained personal assistant, a person must be classified as homebound, which means:

A. A client who has trouble leaving his or her home without help (Such as using a cane, wheelchair, walker, crutches, special transportation or help from another person). Illness or injury means that leaving the home is not recommended.

B. A client who is unable to leave the home and it requires a major effort to do so. The client may leave home for medical treatment or short, infrequent absences for non-medical reasons, like an occasional trip to the barber, a walk around the block, a drive, or attendance at a family reunion, funeral, graduation. 6 Section 1: Medicare Coverage of Home Health Care or another infrequent or unique event. A client can still get home health care if the client attends adult day care or religious services.

Level supervision with guidance from the Health Care Provider or psychiatric Doctor focuses on a treatment plan. (FOT)
